I.C.F (connotes independent living, capability and fulfilling life) is a personalised, goal-oriented and one-stop community rehabilitation service for stroke survivors and caregivers and provided by Community Rehabilitation Network.

Intensive small group training by different health care professionals will be tailor made for stroke survivors aged 60 or below and discharged from hospital within 2 years after stroke in order to maximise the functioning in the active rehabilitation period.  Training includes mobility, cognition rehabilitation, speech and swallowing, emotion, lifestyle modification, community reintegration, vocation rehabilitation and so forth.  Case manager will be delegated to each participant with support from peers for motivation inspitation.
