Please forward the sponsor form with the transaction record(s), “ATM” and “JETCO” transfer slip(s), crossed cheque or original copies of bank-in-slips to the Chest Office or through team coordinator before 19 March 2021 (Friday). Please keep photocopies of all forms and bank-in-slips for verification purpose.
