- Download the enrolment form and email / fax it back to the Chest office (email: dress@commchest.org ; Fax. no: 2506 1201)
A notice will be sent to your Organisation / School by email or fax about the distribution of promotional materials. If you do not receive any information from us two weeks before the event date, please call the Chest office at 2599 6112 / 2599 6124.
For those who enrol before Tuesday, 16 September 2014, the name of Organisation/ School will be listed in our acknowledgement advertisements in major newspapers on the event day.
