Community Care for Patients with Valve Disease

Valve disease is a chronic illness with high recurrence rate and life-threatening consequences.  It is essential to provide accurate and up-to-date information to patients for proper disease management, and to teach them self-care skills for maintenance of optimal health conditions.  It is also necessary to provide emotion and psychological support to patients.

The project aims at providing a comprehensive range of services including health-care talks, establishing rehabilitation programmes, patient support groups, hotline services, etc.
