3C Project (Community Collaborative Care for Mental Health Project)

The project aims at promoting multi-disciplinary Collaborative Care Model together with General Practitioners, Social Workers, Psychiatrist and all private health professions to provide early assessment and short-term intervention for people with common mental health issues at Hong Kong Islands and New Territories West Districts.

Three level of contents will be provided:

• Level 1 : Organising promotional activities for primary care professions, NGOs and public for arising early prevention and help seeking awareness.

• Level 2 : Providing assessment and short-term intervention by Social Workers and medical consultation by General Practitioners, Chinese Medical Practitioners for needy.

Level 3 : Organising body-mind-spiritual mutual support group for participants.
