“Visible Participation" Compassionate Community Action

By increasing the palliative care capacity of home care service providers, the project provides psychosocial care and carer-oriented support service to the elderly with life-limiting illness and their carers in home care service setting, in order to enhance their quality of life.  

Moreover, with the implementation of compassionate community approach, the project aims at re-connecting the elderly and their carers to their support network and the extensive community activities.  It will connect volunteers from various life spans and different community stakeholders in offering meaningful activities for the elderly.
