SafeSmart Living Scheme is a community‑based project in Kwun Tong that adopts an integrated frailty management model to provide one‑stop home‑ and centre‑based support for frail elderly through a multidisciplinary team. The project offers personalised frailty management plans that combine functional and cognitive training, pain management, nutritional support with meal service, psychosocial support, carer counselling, life‑education and end‑of‑life planning, helping them lead a dignity life.
